Employment Application Form

Prior to or after an offer of any appointment, an applicant may be required to undergo a national and/or federal police check. In addition, you will be required to undergo an NDIS Worker Screening Assessment. In accordance with the declaration in the Application Form if a candidate does not meet the requirements of a successful national and/or police check, and/or an NDIS Worker Screening Assessment, the appointment may not proceed or be terminated.

Privacy Policy

Personal information collected via this application and any notes taken at an employment interview will only be used for the purpose of Human Resource Management by TLK. The information you submit to us may be disclosed to referees (with the exception of immunisation records), our team members who are involved in human resource management, recruitment or support roles, security organisations, recruitment agencies and other third parties who assist us in the recruitment process. If you choose not to provide any of the information requested, we may be unable to send you information, fully process your application or properly consider you for employment. Any information we collect about you in the future will be used and disclosed in the same manner as described above unless we tell you otherwise in advance. You may request access to personal information pertaining to yourself that is held by us.

Consent & warranty

I consent to TLK NDIS Disability and Aged Care Services Pty Ltd T/A TLK Care (TLK) using and disclosing my personal information in the manner described above. I give permission to TLK to conduct verification checks on my qualifications with the issuing institutions. I give TLK permission to obtain information from previous employers or listed referees and, if necessary, seek medical information about my previous health from any doctor or hospital or WorkCover. I consent to TLK conducting a Visa Entitlement Verification Online (VEVO) inquiry. I agree to provide TLK evidence of immunisations on request. I consent to TLK holding information provided by me or, obtained about me through third parties (such as criminal record checks) electronically in TLK’s human resource systems. I consent to TLK to keep a copy of personal documentation as proof of my identity for the purpose of this employment. Examples of such documentation include, but are not limited to passport, full birth certificate and drivers licence.

I warrant that:
• I have read and understand the above statement.
• I am qualified to work in Australia, and if requested, can provide evidence of that fact (birth certificate, citizenship certificate, photo ID and/or working Visa, as appropriate)
• All the information I submit (including this form and any attached resume) is true and complete. I understand that any false or misleading information I provide may lead to rejection of my application, review of any employment I accept with TLK and my immediate dismissal from such employment.
• I have disclosed all relevant information in relation to my mental and physical ability to safely carry out the inherent requirements of the position(s) for which I have applied.

TLK provides services to members of the community which may be vulnerable or have a compromised immune system. The safety of our employees and service users is a priority and as such we ask that you answer the following questions honestly. TLK at its discretion can request you provide evidence of immunisation. Failure to do so can result in immediate termination of employment. You do not have to answer the following questions however, if you do not answer we may be unable to fully process your application or properly consider you for employment..

It is essential that you provide the details of two professional referees. At least one of these referees should have been your immediate supervisor in your past or existing role.
